The Dental Implant Surgery Center is open Monday through Friday from 8:00am to 5:00pm. We are also available on Saturdays by appointment only. We will schedule your appointment as promptly as possible. If you are experiencing pain or an emergency situation, every attempt will be made to see you that day! 

Please assist us at the time of your initial visit to the office by providing the following information:

• Your referral slip and x-rays from your referring dentist - unless they have already been forwarded to us.
• A list of medications you are currently taking (please note that you can fill out your patient registration and medical history form on this web page.)
  
• If you have dental or medical insurance, please bring any forms or insurance cards with you to the appointment.

Please Note: All patients under the age of 18 must be accompanied by a parent or guardian at the consultation appointment.

Please notify the office if you have a medical condition or concern prior to surgery (e.g. artificial heart valves or joints, heart murmurs requiring premedication, severe diabetes, or hypertension.)

Because we firmly believe that your time is valuable, we work very hard to honor your scheduled appointment time. If you are unable to keep your initial appointment, we require 48 hours notice to reschedule your visit.
